Job Summary:In accordance with agency policy and state/federal regulations, provides independent, age-appropriate occupational therapy to patients in their place of residence. Teaches patient/family and caregivers preventative and rehabilitative procedures. Develops, coordinates and manages the plan of care.Essential Responsibilities:
Evaluates patients and interprets evaluation findings to determine the nature and extent of dysfunction
Comprehensive evaluation and ongoing assessment of patient competency in ADLs, IADLs and underlying skilled set
Detects early warning signs of complications including medical assessments of skin integrity and mentation
Ability to differentiate emergent versus non emergent patient care status and escalates appropriately
Providing initial and ongoing assessment to determine level of functioning, including OASIS assessment at appropriate time and point
Evaluate the patient comprehensively, not just body part or functional ability
Establishes treatment goals and plans treatment to achieve established goals
Initiates discharge planning for occupational therapy
Implements the plan of care through direct treatment of the patient, delegates a portion of the treatment plan to appropriate supportive personnel, and coordinates the treatment program with other health care personnel and home health team members as appropriate
Provides advice and education to patient and family
Regularly reassesses clinical signs and symptoms to determine effectiveness of treatment, progress of patients towards goals, and the need for modifying goals and treatment
Discharges patient from occupational therapy when the patient has met their potential in the home health setting
In accordance with agency policy, provides accurate, timely documentation of occupational therapy patient management
Participates in appropriate and approved educational and clinical research activities in occupational therapy
Completes OASIS documentation according to guidelines
Coordinates with pharmacy, inpatient and outpatient rehab departments
Assists in the development, implementation, and review of departmental patient care programs
Performs other duties as assigned
Supervises the work of Home Health Aides according to regulation and agency policy
Supervises the work of Certified Occupational Therapy Assistant according to regulation and agency policy
Demonstrates on-going progress toward meeting the criteria for professional practice
Consistently supports compliance and the Principles of Responsibility (Kaiser Permanentes Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanentes policies and procedures
Basic Qualifications:
Experience
Minimum two (2) years of experience as an Occupational Therapist (OT).
Education
Graduate of an accredited occupational therapy curriculum with a bachelors or masters degree with certification in occupational therapy OR foreign-trained equivalent.
License, Certification, Registration
National Provider Identifier required at hire
Basic Life Support required at hire from American Heart Association
Additional Requirements:
Current CA Occupational Therapist License Or: License eligibility 120 days from the the letter date to pass the OT licensing exam.
Must be able to work in a Labor/Management Partnership environment
Clear driving record for the past two (2) years and proof of automobile insurance as required by law
Computer literate and proficient in electronic documentation
Preferred Qualifications:
Experience in an automated clinical information system preferred
One (1) year experience in home health agency preferred
