Must be knowledgeable in areas such as active listening, social perceptiveness, critical thinking, and coordination. The social worker has the ability to carry the role of advisor, therapist and clinician.

Conducts psychosocial evaluations of potential recipients to identify barriers to candidacy for LVAD ensure lack of coercion and address psychosocial risks. Encourages participation in decision making by the patient. Ensures that the patient has adequate electrical and phone services as well as a safe environment for a LVAD. Provides formal recommendation to the MDT as to whether the patient is appropriate for LVAD

Attends inpatient rounds for all MCS devices to assist in ensuring goals of care are understood and family or caregiver understanding is optimal.

Assures completion and appropriateness of complex discharge plan and communicates the plan of care to the team. This may include but is not limited to obtaining DME, setting up infusion therapy, home tube feedings or skilled nursing.

Collaborates with interdisciplinary healthcare team members to ensure continuity and coordination of patient care throughout the continuum and into the community by coordinating placement with home health, long term acute hospitals (LTACs) and skilled nursing facilities (SNFs), assisted living, acute rehab, and psych placement. Assists team in evaluating appropriateness of facility.

Provide liaison services and referrals to hospital resources and community agencies, including medication assistance programs.

Attends the outpatient clinic to obtain updates on psychosocial status, provide crisis intervention and to facilitate referrals for necessary services.

Monitors and documents patient adherence with compliance contracts. Pays close attention to those who are at a higher risk for morbidity and mortality.

Leads Education Classes and Support Groups by reviewing requirements for transplant, coping with complex medical conditions, caregiver topics, financial concerns and substance abuse relapse prevention.

Counseling patients suffering from adjustment issues related to chronic illness, life adjustments with the LVAD, guilt in moving forward with transplant.

Remains current of Medicare and Medicaid laws and guidelines, Medicare Conditions of Participation, managed care contracts, and other legal and regulatory requirements as they relate to health care social work services. Assures patients are informed of rights and responsibilities as participants in these programs

QualificationsMinimum EducationMaster’s Degree from a Council on Social Work Education accredited graduate schoolMinimum Years in Field2-3 years clinical social work or allied health practice, with hospital or medical background or experiencepreferred

Current Arkansas License

Master’s Degree from a Council on Social Work Education accredited graduate school

LCSW required or eligible for

Mechanical Circulatory Support, Transplant, Dialysis, Psychiatric and/or PM&R experience a plus
