JOB REQUIREMENTS
Education
Licensed by Arkansas State Board of Nursing as an Advanced Practice Nurse in (Palliative Care/Adult Gerontology) Certification by national certifying body with prescriptive privileges.
Experience
Three (3) years in healthcare setting desired.

Provide healthcare services for patients under the St. Bernards Palliative Care Program. Patient care will be delivered in various settings including: St. Bernards Outpatient Palliative Clinic, St. Bernards Cancer Center, Encompass Rehabilitation Hospital, LTACH, Home-based care among others. Performs professional duties in collaboration with the Palliative Care physician as follows: Assessment of health status of clinic patients; Formulates an appropriate differential diagnosis; Develops and carries out a plan of care with the inter-disciplinary team. The APRN may be asked to assist with acute patients in the Senior Health Clinic.
JOB SUMMARY
Provide healthcare for patients in various health settings. This will include St. Bernards Senior Health Clinic, Cancer Treatment Center, home setting, long-term acute care facilities, inpatient rehab facilities and any other facility where palliative services may be needed. Performs professional duties in collaboration with Geriatrician and/or Palliative phyisician as follows: Assessment of health status of clinic patients; Formulates an appropriate differential diagnosis; Develops and carries out a plan of care in collaboration with enter disciplinary team.
