We seek an experienced FNPto join our primary care team, dedicated to providing value-based care through a virtual telehealth platform. This innovative role emphasizes a team-based approach, collaborating closely with our population health team, practice manager, and support staff. The successful candidate will play a crucial role in closing care gaps and ensuring our network members receive comprehensive, high-quality care.Key Responsibilities:
Provide primary care services via telehealth to patients within Banner plans and network
Collaborate with established primary care providers to ensure continuity of care.
Work with the population health team to address care gaps and improve patient outcomes
Focus on value-based care quality measures and population health initiatives
See an average of 16-18 patients per day
Qualifications:
Must live in the Phoenix valley for occasional onsite functions
FNP designation with AZ license eligibility
Minimum 2 years of clinical experience as an FNP in a primary care setting
DEAwith prescriptive authority required
Passion for value-based care and population health
Comfortable with telehealth and virtual patient interactions
Patient-focused, friendly, and willing to collaborate in a team environment
