Integrated Health and Outreach Program Coordinator

The Integrated Health and Outreach Program Coordinator manages an integrated clinical and community-based program, including screening, testing, treatment, and referral for HIV, hepatitis C, sexually transmitted infections, other transmittable diseases, and substance use disorders. The coordinator provides overall administrative management, direction of program activities, and ongoing support for clinical software applications in electronic medical record systems.KEY ACCOUNTABILITIES:Provides direction of program deliverables to meet goals and objectives in alignment with NATIVE HEALTH’s mission.Provide administrative oversight of the program.Coordinate integrated services with clinical staff, behavioral health department, other departments, and partners.Support the capacity of clinical software applications to improve service delivery of health and program services.Supports the organization as needed.
