LCSW
Master’s Degree in related field (The exception of those who were licensed prior to 2004 but are currently licensed independently is acceptable). - If they aren’t licensed, the degree must be Master of Counseling or Master of Social Work
2 years related experience in behavioral healthcare, physical health, or substance abuse.
Associate or Independently Licensed Professional with The Arizona Board of Behavioral Health Examiners as Psychologist, Social worker, Counselor, Marriage and family therapist or Substance Abuse Counselor
