The Home Alliance Coordinator (HAC) provides services to Hope support recipients through their leadership of an Assisted Living Home. The HAC is responsible for the overall management of the assigned home. The HAC ensures the health and safety of support recipients and provides a wide range of services. This position includes teaching of living skills, social and communication skills, and other needed life skills for social independence and community inclusion. The HAC facilitates communication amongst all members of the support recipient’s team. This position leads and supervises the team of direct support employees who work in the home, ensuring the implementation of the Plan of Care and/or support plan. This position requires that the HAC live in their assigned Assisted Living Home. The HAC is responsible for being in the home unless accompanying a support recipient in the community, conducting Agency business, on approved time away, or on a designated day off. The Home Alliance Coordinator maintains loyalty to the organization, its beliefs, values, mission, vision, and expectations, and to the recipients of services.

Essential Job Duties

1. Provides overall leadership and management of the Assisted Living Home.

2. Supervises, develops, evaluates, counsels, and disciplines staff through teaching, coaching and mentoring. Collaborates in all aspects of onboarding personnel to fulfill department requirements.

3. Shares all pertinent information with the Relief Individual Support Specialist before taking time off.

4. Operates the home within Hope’s, and each support recipient’s, budgetary guidelines, including responsibility for staff allocations in alignment with Hope’s budget. Responsible for the home and support recipients’ funds and monthly reports.

5. Ensures wholesome meal preparation, focusing on individual dietary needs and healthy personal and cultural preferences.

6. Ensures appropriate cleanliness, maintenance, and safety of physical site (home and grounds) and home vehicle.

7. Assists in the development of Plans of Care and/or support plans and interaction guidelines.

8. Provides, or schedules staff to provide, personal care and support services required for each support recipient, including, but not limited to, activities of daily living and community activities. Ensures each individual participates safely in a wide range of age-appropriate activities. Ensures goals and objectives are being worked on as outlined in the Plan of Care and/or support plan.

9. Coordinates, and provides if necessary, transportation to meet support recipients’ schedules, including public transportation and rides to work, appointments, and activities.

10. Monitors support recipients’ health and ensures each support recipient follows medical orders and medication instructions, under the direction of the support recipient’s physician and/or Hope nursing staff. Accompanies and participates in medical appointments as necessary. Accesses emergency services as needed.

11. Facilitates, coordinates, and/or participates in home, behavioral health, and other team meetings occurring during work days, as required. Provides training for staff in the home as assigned.

12. Verifies, approves, and quality assures time sheets, daily log notes, service notes, goal tracking, and other documentation generated by staff working in the home.

13. Completes and processes Agency documentation within required timeframes, including the timely submission of all maintenance requests, vehicle safety checklists and accident reports, monthly home reports, and incident reports. Regularly reviews and/or quality assures documentation to comply with Agency standards.

14. Maintains Home License and requirements for re-licensure, in collaboration with the Hope Assisted Living Home Supervisor and Adult Licensing Specialist.

Education and Experience Requirements:

1. High School diploma or GED required. Bachelor’s degree in Human Services, Health or Social Science, Special Education or other related field preferred. Emphasis of study in developmental disabilities preferred.

2. Two years of experience providing direct support services required; one year of supervisory or administrative experience strongly preferred.

Additional Requirements:

1. Must be 21 years of age or older.

2. Meets State and Agency requirements for the Administrator of an Assisted Living Home.

3. Lives in the home and provides ongoing supports 24 hours per day as needed, with the usual schedule of 16 hours of work and eight hours of sleep per day. This schedule may be adjusted based on the support recipients’ needs and the home staffing allocation.

4. Possesses a valid Alaska driver’s license and has at least one year of driving experience (may include permit time). Obtains a driving record at time of employment, annually thereafter, and as requested by management.

5. Ability to safely operate Agency vehicles when needed. Possesses reliable transportation and vehicle insurance when transporting support recipients or conducting Agency business.

6. Meets State and Agency requirements for passing a criminal background check upon hire and as required thereafter.

7. Lifts 35 pounds from a squat and kneel position on the floor.

8. May be required to frequently walk, bend, stoop, kneel, reach, pull, and/or push.

9. Completes all required State, Agency, department, and position specific training as assigned.

10. Obtains a tuberculin clearance at time of employment and annually thereafter.
