Interview patient to determine medical history, condition, desired results and appropriate modality.
Assess quality of patient care and coordinate patient care services with patients, staff, physicians, and other departments.
Educate and counsel patients, caregivers and family members on treatment options, placement in alternative care settings, and continuing care.
Document patient information, interview responses, treatments, feedbacks and results.
RequirementsEducation:
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
