Arranges and coordinates scheduling of all HHA/PCA and Nursing assignments in consultation with Nursing Services Director and/or administrators, assuring coverage of all assignments.

Creates, up-dates, and maintains all HHA/PCA and Nursing schedules.

Notifies staff of schedule requirements and changes.

Notifies nurse supervisors and administration of scheduling, including problem areas.

Maintains established on call staffing par levels.

Maintains Staffing communication documentation.

Provides direct marketing through telephone communications and appropriate follow-up.

Uses correct telephone skills during intake procedure.

Maintains staffing and recruiting documentation.

Maintains files for all pending cases.

Generates referrals by networking via the telephone with referral sources. Ensures the delivery of quality care services by maintaining open communication with clients, families’ staff, and by adhering to agency policy.

Recommends, schedules or completes nursing assessments.

Alerts supervising nurse to potential problems.

Maintains on call sheet.

Maintains current and up to date availability tracking tool for Staff.

Recruits HHA / PCA and Nursing staff in conjunction and accordance with the Atria Corporate HR Department.

Supplies and assist with and collects completed application package from applicants.

Verifies submitted certifications as appropriate.

Obtains, verifies, and documents references in writing and/or via telephone.

Maintains appropriate application files and/or logs in compliance with Atria Corporate HR policies.

Assists in arranging interviews of candidates by Nursing Services Directors.

Submits appropriate and completed application packages to Agency Business Director (ABM) for approval prior to hiring.

Assists in obtaining fingerprints as appropriate.

Assists in pay roll function as appropriate/assigned by Agency Business Director (ABM).

Supervises, develops, and schedules the client services staff according to agency policies, procedures and standards.

Establishes and maintains a medication system which adheres to state, federal and company laws, regulations and guidelines.

Performs assessments and reassessments to determine client needs.

Develops appropriate Service Plan to meet client’s needs while ensuring safety and support for client independence. Consults with physician and receives physician approval and orders for Service Plan.

Instructs staff regarding client’s needs and Service Plan. Ensures that service plan is being followed and documentation is completed as necessary.

Operates the within budget and according to agency’s policies, procedures and standards.

Completes weekly Quality Enhancement reviews to ascertain if the agency is meeting state regulations and agency policies.

Act as a consultant and resource for issues related to client care within the community.

Ensures adherence to company policy and State Health and Safety Codes, and federal regulations for licensed home care agency.

Administers medications as allowed per state specific guidelines when required.

Maintains skills in client care and clinical function.

Maintains medical records and complete paperwork in compliance with regulations and agency policy.

Provides activities of daily living assistance to the clients as needed.

Provides training and orientation to new staff members and ongoing training to current staff members.

Communicates to clients, families, department heads, staff, and Executive Director in an effective and respectful manner.

Delivers and attends required training for self for professional licensure and agency.

Maintains positive relations with clients’ families and physicians.

Maintains active community and professional ties.

May perform other duties as assigned.

A registered nurse or licensed practical nurse licensed to practice as required by New York state regulations.

Experience in direct patient care and assessments/observation of clinical conditions.

Experience in recruiting, training and managing people in the healthcare field.

Experience at a supervisory level in homecare.

CPR certified and knowledge of basic first aid.

Experience coordinating educational events.

Must successfully complete the Atria Medication management training, and all agency specified training programs.

Must complete all state required training to maintain licensure or certification.

Able to work flexible work hours due to demands of position, including weekends.
