Community Health Assistant

The Idaho Healthcare for Children and Families AmeriCorps Program is currently seeking a motivated individual to serve full-time for one year as a Community Health Assistant at Genesis Community Health, in Boise (dependent on grant funding).
To be eligible:
Must be 18 years of age or older.
Must be a US Citizen or Lawful Permanent Resident.
Be able to complete 1700 total hours over one year. Start date September 1, 2019
Previous experience or knowledge in mental health, crisis intervention and or suicide prevention.
Pass a Background Check.
Complete ISPH phone responder training.
Have a Drivers License & Reliable Transportation for minimal travel.
Undergraduate student in Social Work, Public Health, Health Education, Health Sciences, or related degree.
A strongly self-motivated person able to succeed with minimal resources and supervision.
Strong ability to communicate well, both with individuals and various group sized settings, including public speaking.
Ability to lead and direct the work of others, including multiple volunteers.
Ability to work in a multi-disciplinary team environment of medical, dental, community and mental health professionals
Heart to serve the underserved with dignity and respect.
Proficiency in Microsoft Office products
Experience using an EMR and/or the ability to become proficient with one
Ability to apply the Mission of Genesis to daily work
Responsibilities:
Coordinate social, medication assistance, specialty, and counseling services for patients in a holistic community health care setting.
Work closely with the Community Health Coordinator and VPN/MAP Coordinator, organizational leadership and multi-disciplinary team to develop and implement policies, procedures and processes related to social services, mental health, medication assistance, and specialty care for patients.
Build and maintain partnerships with other social service agencies, counselors, schools, other medical providers and churches to be able to refer patients.
Assist with managing volunteers initiating health risk intake in clinic to ensure coverage.
Work with clinic team regarding patient service issues.
Follow-up on patient charts to ensure those referred for counseling, medication assistance, specialty care, or various social services are being connected to service.
Complete individual appointments with Genesis patients needing additional social service assistance and education.
Keep Community Health resources and box up to date and supplied.
Attend community health events as needed.
Assist with the implementation and integration of medication assistance software

Living stipend of up to $13,992 paid over the contracted term
