Physical Therapist />Medford, Wisconsin /> />myPTsolutions is currently looking for a Physical Therapists to hire for a 3-month travel assignment in Medford, Wisconsin. The coverage area includes Medford and approx. A 60-mile radius around the city. Possibility to treat patients in Wausau east to Tigerton, south to Mosinee. Home Health experience, training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnoses. Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy. /> />

Responsibilities of the Home Health Physical Therapist />Evaluate and assess your patients to establish functional, measurable goals that promote restoration of functional abilities. />Realize your own potential to achieve your goals and overall excellence. />Advise and consult with the family and/or other caregivers as indicated to promote the client' s level of progress toward mutually agreed upon goals and plan for discharge. />Participate in educating the client and the family or other caregivers to promote the client' s progress toward mutually agreed upon goals. /> />

Requirements />Meets productivity expectations of an average of 4.5 visits per day />Minimum 1 year of Home Health PT experience - Required />AHA BLS active at submission - Required />Wisconsin State PT License />Masters Degree from PT Accredited College />AHA BLS />NetSmart EMR /> />
