ESSENTIAL FUNCTIONS: (Functions that is fundamental for the performance of the position. List essential functions comprising the job in order of importance and percentage of time spent on each.)
1. To provide necessary assistance to residents as follows:
Dressing/undressing Grooming
Bathing Toileting/elimination
Personal Hygiene Health and Wellness Behavior
Movement Nutrition
Psychosocial care Spiritual care
2. Effectively read, interpret, and act upon nurses instruction(s) as notated in each resident service plan.
3. Monitors and documents resident’s body functions: Urine, Bowel Specimens, Vital Signs, Height and Weight.
4. Observe and report resident’s condition to appropriate staff, receive and share information, as well as, work with team members in a helpful respectful manner. Report to Management and AMAP any deviation of personality traits.
5. Maintain safe work practices and follow facility safety procedures.
6. Satisfy continual training requirements for in-service training.
7. Attend any meeting(s) scheduled by facility administration.
8. Attend any meeting(s) required by facility administration, any state agency, and any federal agency that are called in order to meet any documented requirement(s).
9. Maintain required certification/qualifications and associated standards.
10. Completion of tasks outlined on one of the three or four different Shift Check Sheets (dependent upon which shift is being worked). See attachments for details.
11. Accountability system participation. As you enter a shift, you are to verify completion of tasks for the previous co-worker. As you leave your shift, the in-coming co-worker will verify completion of tasks as outlined on your current shift check sheet.
12. Compliance with the Pre-employment and Employment Check Sheet.
13. Other duties, as assigned by Administration.
