We are a leading Medicaid Home Health Agency dedicated to providing Caregivers to our clients in the comfort of their own homes. We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team. The successful candidate will be responsible for conducting assessments, and coordinating with other healthcare professionals to ensure the correct assessment information.
Responsibilities:
Monitor clients' progress and make necessary adjustments to the plan of care
Communicate effectively with clients, families, and caregivers to ensure understanding of Client's diagnostics
Drive to clients' homes to perform assessments every 1 or 3 months, as required by Medicaid regulation
Conduct initial and ongoing assessments of clients' health status
Requirements:
Valid RN license, with certifications up to date
Reliable transportation and willingness to travel to clients' homes for assessments
Excellent communication and interpersonal skills
