Social Work SupervisorJob DescriptionPurpose & Scope: Reporting to the Director of Case Management, the Social Work Supervisor will supervise Bachelors prepared Case Management staff and be a resource to the case management team on complex discharge planning.The supervisor will carry their own cases in addition to severing as supervisor for Bachelor’s prepared Case Mangers and an escalation point for complex cases as needed.Education: Master's in Social Work is required.Experience: Two years of hospital case management experience to include but not limited to the following patient populations: ER, labor and delivery, post-partum, ante-partum, NICU, trauma, med/surg. Certification/Licensure: None
