The LPN / LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.Requirements

Two years' experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Hospice experience highly preferred.

Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.

Home Care Home Base knowledge highly preferred

Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.

Coordinate care with the interdisciplinary group, patient/family, and referring agency.

Assist in the development and implementation of patient care plans.

Monitor patient health and report any changes to the supervising RN.

Administer medications and treatments as prescribed.

Educate patients and their families on health care needs, conditions, and options.

Maintain accurate and timely documentation of patient care and progress.

Graduate of an accredited School of Vocational/Practical Nursing.

Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.

Reliable transportation and valid, current auto liability insurance.

Work in patients' homes in various conditions.

Ability to work a flexible schedule and travel locally.

Ability to lift up to 50 lbs. and move patients.

Ability to make quick decisions and acquire necessary resources.

Must meet patient/family individualized psychosocial needs.
