Established home health agency with Medicare & Medicare Advantage home Health program is looking for an Administrate Assistant (intake Coordinator) in our De-Soto Office or Dallas office or Bonham Office. The home health agency has several locations. We had been in business for over 30 years. We provide Home Health to Medicare & Medicaid Clients, Medicare Advantage, Insurance, VA clients, PHC & private pay clients including RN, LVN, PT,OT, SP, MSW, HHA, Attendant, Home Maker.

Challenges:

Home health is very challenging and demanding job. You will be very busy with several tasks & duties. You have to work at very fast phase. You have to very detail oriented and good memory. Do everything logically. Each patient care is different from one patient to another.

JOB RESPONSIBLITIES:

1. Data entry of new referrals sends to us by physician office or hospital discharge planner.

2. Verify the patient insurance eligibility, determine weather we have all documents of referral to be accepted & admitted.

3. Enter client demographic in Kinnser soft ware

4. Report to DON about the referral,

5. Schedule the client assessment & admission by an RN.

6. Schedule LVN visits

7. Refer the clients to PT, OT, SP Company for therapy treatments.

8. Conduct care coordination of the client from referral to discharge from our services when goals met.

9. Comply with all CMS, & THHSC regulations and do everything legally.

10. Get authorizations from insurance company.

Basic Requirements:

Must have an Associate Degree in health care or business related program from a college.

Must have at least two years hands on administrative experience in Home Health Office experience working in the office doing administrative tasks.

Must be self starter and leader able to work with minimal supervision

Must be very fluent in verbal & Written Communication skills.

Must have excellent skills in Word Processing, Excel , and prefer use of VESTA software.

Must be a fast worker with good memory.

Must be very organized.

Must have very good attendance.
