Using standards, you will manage benefits and justify service necessity. This role combines healthcare practitioner, case manager, and educator duties to achieve top clinical outcomes. Assess the patient's care plan. Document, execute, and update collaboratively with the healthcare team to deliver evidence-based care.
Construct patient care management strategies, treatments, and self-management priorities.
Evaluate the healthcare, educational, and psychosocial requirements of the patient and their family.
Help patients manage health autonomously through education, in-person visits, and remote engagement. Encourage patients to follow care plans.
Explore, assess, recommend resources for medical and non-medical needs of patients and families.
Unite with all programs for service coordination.
Identify and address problems in the care coordination, exploring potential solutions to ascertain the most suitable care setting.
Serve as a liaison to hospitals, general care providers, post-recovery partners, and specialists.

Proficiency in conducting patient assessments and managing their care is critical for this role.
Exceptional verbal and written communication skills are paramount.
They should possess the ability to diligently monitor, evaluate and document patient progress with respect to personalised care plans.
They should have the capability to facilitate a patient's access to appropriate community resources.
We must work well with internal medical team and external partners and organizations.
Together, successful partnership is crucial.
They must have a flexible capability to evaluate, adjust and respond positively to a shifting and crisis-filled environment.

Must hold at least an Associate's degree.
Should have a minimum of two years relevant work experience.
Holds a valid Registered Nurse license.
