You will be involved in launching a new Program of All-Inclusive Care for the Elderly model-centered on the belief that the well-being of seniors with chronic care needs and their families are better served in the community. Bring your expertise and skills as a member of the IDT to help manage the complex medical, functional, and social needs of older adults as we implement our Northern Kentucky PACE program.
