Shifts Available: Day shift, Full Time Position.

General Responsibilities:

Observes and reports the social and emotional factors relating to the client’s condition, need for care, response to treatment and adjustment to care.

Plans, organizes and implements physical therapy interventions according to prescribed therapy, initially and at each recertification period.

Evaluates the physical therapy plan of care.

Evaluates any deficits in independent living/daily living skills, sensor motor skills, cognitive skills and psychosocial skills.

Evaluates client’s environment and activity and determines if architectural barriers should be removed or if assistive/adaptive equipment is needed.
