Conemaugh Nason Medical Center is seeking a PRN - Social Worker. The PRN - Social Worker is responsible for coordinating the level of care appropriate to patient needs in collaboration with the Case Management Team. Performs discharge planning based on high-risk assessment screening. Works with patient/family/physician to execute discharge plan. Provides direct intervention and expertise to the care team related to psychosocial support for assigned patient care areas. Utilizes the high-risk screen in the initial discharge planning evaluation
Evaluates all referred patients for discharge planning needs within one business day of receiving notice of the consultation.
Collaborate with an interdisciplinary team to determine appropriate care coordination, resource utilization and discharge planning.
Provides and documents freedom of choice to patient and family as indicated.
Provides guidance to the multidisciplinary team related to patients’ psychosocial needs.
Interacts with external facilities to ensure timely discharge to appropriate location including but not limited to skilled nursing, personal care, rehab, other acute care setting or home.
Facilitates discharge plans for high-risk cases and completes appropriate documentation in medical record.
Keeps abreast of and makes referrals to various community resources (Federal, State, Local and others as appropriate) to meet patients’ needs.
Provides discharge-planning support to various outpatient departments in the hospital as assigned.
Identifies potential home health patients and makes appropriate referrals to home health agencies as needed.
Provides crisis intervention as necessary for patients and families including Trauma, suspected abuse, deaths, codes, children/youth issues or terminal illness.
Responds to trauma activations in the DEM as assigned.Organizational Expectations:

Education:Minimum Education
Bachelor’s Degree in Social Work required.Minimum Work Experience:
2 years’ Acute care hospital experience is preferred.Required Skills:
