The Home Care Quality & Safety Program Manager supports the CHOP mission of delivering the very best healthcare to children. The Program manager is responsible for planning, coordination and facilitation of patient safety and quality improvement work that supports the Home Care Department' s initiatives as well as regulatory requirements that impact patient outcomes, enhance value, build efficiencies and minimize patient harm. Provides coordination and facilitation of the collection, analysis, trending and benchmarking of data and outcomes across the department. The Home Care Quality and Safety Program Manager collaborates with Home Care Leadership and partners with safety and quality programs for the CHOP enterprise to ensure system-connectedness, alignment and consistency.What you will do

Collaborates with the Center for Healthcare Quality & Analytics to support and interface the Home Care quality improvement plan and initiatives with the organization's overall performance improvement and innovation initiatives.

Participates in prioritizing improvement actions and works with the Home Care Leadership Team to identify and engage the key stakeholders with accountability to pursue these improvements. This is achieved through participation in various department & campus based and hospital committees, related to KPIs, quality, safety and data analytics

Facilitates quality improvement efforts and teams with a goal of achieving the identified improvement aim and building organizational capability through:

Using the CHOP Improvement Framework and/or other methodologies as a means of making improvement

Demonstrating expert facilitation for clinically and/or operationally focused chartered improvement initiatives

Promoting and engaging in performance measurement as a guide for improvement

Supports the development of measures and designs data collection methods for initiatives and projects.

Assures implementation, accurate data collection, and timely monitoring of data outcomes.

Collaborates with the Home Care leadership team on the evaluation of the Home Care Department's quality and patient safety program for patient safety quality improvement initiatives and projects.

Assures that performance improvement initiatives and data quality controls are in place to monitor the home care services and programs as it relates to performance, professional standards of practice and regulatory requirements. Utilizes evidenced based research when developing initiatives, outcomes and programs. Benchmarks internal and external best practice in the planning and implementation of performance improvement initiatives.

Responsible to assure that the Home Care leadership team is directly involved with the data outcomes and implementation plans as required. Provides consistent reporting to department, education for improvement concepts or specific pl n, and champions' improvement work to improve patient outcomes.

Responsible for components of the Home Care Annual Program Review

Promotes successful organizational regulatory compliance through the coordination of home care activities, including tracers, data reporting and formal and informal staff education.

Active participation in any regulatory review (JC, DOH, CMS, FDA)

Collaborates with Home Care Leadership on the attainment of the annual operating plan for the Home Care DepartmentEducation Qualifications

Bachelor's Degree Healthcare Profession Required

Master's Degree Healthcare Profession Preferred Experience Qualifications

At least five (5) years hospital or home care-based experience in one the following areas: Patient safety, RCA, FMEA, clinical process improvement, or healthcare quality improvement Required Licenses and Certifications

Certified Professional in Healthcare Quality (CPHQ) - National Association for Healthcare Quality - upon hire - Preferred or

Certified Manager of Quality (CQM) - National Association for Healthcare Quality - upon hire - Preferred or

Lean Six Sigma Green Belt (LSSGB) - American Society for Quality Control (ASQ) - upon hire - Preferred
