Permanent Supportive Housing (PSH) Case Manager at NWPP

We are hiring a PSH Case Manager to join our growing non-profit! NWPP is a fun and supportive organization that provides meaningful impact within the Portland community.

Job Duties:

Maintain a caseload of approximately 15 very low-income seniors who have experienced chronic homelessness.

Work collaboratively and respectfully with clients to develop effective housing plans to address immediate and long-term housing and healthcare needs.

Meet regularly with clients, primarily in community-based settings and always with a strengths-based and client-centered approach, to assist with access to resources and services that support their housing and health stability.

Provide assistance with housing applications, advocacy with landlords, move-ins into permanent housing, and/or eviction prevention services.

Job Qualifications:

Minimum of one year experience providing social services to low-income households.

Interest in working directly with people who may be experiencing a variety of challenges, including poverty, aging, homelessness, and significant health issues.

Willingness to engage in meaningful training, self-examination, and action directed at promoting diversity, equity, and inclusion in the organization.

Excellent interpersonal skills.

Experience working with computers, databases, and commonly used software.

Preferred Qualifications:

Demonstrated understanding of the root causes of homelessness and unique challenges faced by seniors of color and other historically marginalized communities

Demonstrated ability to provide culturally responsive services to vulnerable populations

Bachelor’s degree from an accredited college or university is preferred
