CommonSpirit Health is committed to creating healthier communities, delivering exceptional patient care, and ensuring every person has access to quality health care.ResponsibilitiesAs an entry level clinician, the Clinical Dietitian I assesses thenutritional status of the patient using recognized tools of assessment,and recommends and counsels patients, family, and/or caregivers onthe appropriate nutrition plan to improve the patient's disease orcondition management. Works with general patient population, as wellas, with patients in more complex level of care under supervision of aClinical Dietitian II or Clinical Dietitian Lead.Job Responsibility 1Performs nutritional assessment using age and gender appropriatestandards, cultural, religious and ethnic backgrounds; and utilizingvalidated assessment tools resulting in nutrition related diagnosis whenindicated. Conducts routine screening to identify patients at nutritionalrisk. Collaborates within an interdisciplinary environment to contributeto facilitating care and identifying patients at nutrition risk or needinghigher level of care.Job Responsibility 2Implements targeted and specific nutrition intervention(s). Providescustomized nutrition education to patients and/or families andcaregivers. Works within an interdisciplinary environment to contributetoward overall patient care. Closely monitors, evaluates and rescreenspatient nutrition status as appropriate. Updates plan of care asnecessary. Follows up with patient as needed.Job Responsibility 3Develops an intervention plan which includes appropriate feedingroute, mode, and nutrition prescription unique to patient's specificneeds, in accordance with physicians' orders. Monitors and evaluatespatient response to interventions by meeting specific measurable goalsdocumented in the nutrition care plan in a timely and thorough mannerin the medical record.Job Responsibility 4Collaborates with the Food and Nutrition Services team to enhancepatient experience. This includes participating in test trays, menureviews, periodic auditing trayline for accuracy of diet orders, validatingmenu compliance with the facility Diet Manual Handbook and NutritionCare Manual (NCM), supporting with patient interactive positions (i.e.dietitians, diet clerks, etc.), maintaining communication with patients,staff, and guests and providing excellent customer service. Participatesin peer reviews and chart audits, and collaborates in Diet ManualHandbook training for all clinical staff (new registered dietitians, nurses,therapists, physicians, etc.), including how to access it in hard and softcopy.QualificationsEducation Bachelors Degree with completion of accredited dietitian program(ACEND - Accreditation Council for Education in Nutrition Dietetics.)LicensureValid registration as a Registered Dietitian with the Commission onDietetic Registration (CDR).
