Provides outreach and enrollment, develops Plans of Care and conducts continuing care management services with children and families
Coordinates, identifies, and secures services relevant to the Plan of Care
Completes documentation with a high level of quality and detail to meet and exceed Health Home, and Department of Health requirements
Participates actively in trainings, meetings, and functions at the agency and in the community
Provides quality direct Care Management services to children and families in home, school, and community settings
Uses a strength-based approach to arrange for, educate about, and manage the coordination of medical care and other services as needed
Provides on-call availability to Health Home Program as assigned
Interfaces and networks with partnering Health Homes, providers, and agencies
Must obtain certification to administer CANS-NY within 90 days of employment; certification must be continuously maintained while in the position
