The Case Manager promotes client wellness and autonomy through advocacy,

Must have a BA degree
Assessment, planning, communication, education, resource management, and service facilitation.
Meet with caseload clients weekly and as needed on an individual basis,
through assessment with supervisor.
Link clients with appropriate providers and resources throughout the continuum
of health and human services and care settings based on the needs and values of the client.
