Job Title: Home Health Nurse/Case Manager Travel Required

For this role you will be required to travel to patients residences as a visiting nurse. This is NOT remote or hybrid.
About the Role:
We are seeking an experienced and compassionate Home Health Nurse to join our team, working with adult and geriatric patients. The ideal candidate will have a strong understanding of Managed Long-Term Care (MLTC) and community health nursing, with a focus on creating patient-centered care plans and achieving positive clinical outcomes.
Key Responsibilities:
Develop and maintain comprehensive, interdisciplinary home health care plans in collaboration with patients, families, and physicians.
Regularly update care plans to reflect changes in patient needs and goals.
Case manage care and services to achieve positive clinical outcomes and minimize rehospitalization risks.
Support agency financial goals through effective resource utilization and cost management.
Demonstrate expertise in community health nursing, providing care that aligns with best practices and regulatory standards.
Qualifications:
Education: Associates or Bachelor of Science in Nursing (BSN) preferred.
Licensure: Current LPN or RN License
Minimum one year of acute, Medical Surgery Experience; CHHA/LTHHCP exp preferred:
Familiarity with community health nursing is required;home care and geriatric nursing is a plus.
Excellent clinical assessment, case management, and communication skills.
Proficiency in interdisciplinary care planning and collaboration.
