Req Number 118269Job DescriptionUnder the supervision of the Director and hospital staff, works to support the hospital -based violence intervention program (HVIP) and provide assistance to victims of violence in the hospital. Primary role is to support the development, implementation and expansion of the hospital-based violence intervention program, including coordinate program operations and logistics among hospital responders, and work to improve provision of resources and support services to those injured by violence. This role will also require some familiarity with program implementation and evaluation, to document and track program progress and relevant data related to program implementation. This position will involve engagement with various stakeholders including patients, families, community organizations, local businesses, clinical and non-clinical healthcare providers, and religious organizations for resources, services, outreach and special events. Works in the development and coordination of the peer education program within established program guidelines including work with peer educators to create outreach and education opportunities throughout the entire service area, developing and providing public education, awareness and active outreach in violence prevention and intervention, leading to an increase in prevention and harm reduction. Provides the community, patient and families, and internal and external teams with resources that will assist the victims of crime, save lives and improve community safety by changing behaviors, attitudes and social norms related to gun violence.Job Responsibility1.Supports the development, implementation and expansion of hospital-based violence intervention programming.2.Conducts thorough needs assessment with each HVIP patient, and determine opportunities for assistance. Provides services and support to those injured as needed.3.Provides psychosocial support services to assist patients and families impacted by gun violence.4.Evaluates, plans, and monitors project implementation for clinical systems; manages project and project delivery teams, ensures deadlines are met, escalates issues for appropriate, timely resolution and adheres to standardized project management methodologies.5.Collaborates with interdisciplinary project teams in coordination with community to ensure deployment of systems; serves as liaison to project sponsors, project teams and stakeholders; works with internal and external groups to optimize success of project deployment.6.Supervises and communicates with departments and project teams on project activities; confers with project staff and stakeholders to provide support and resolve problems; collaborates with clinical, program, financial and quality teams to define and develop metric goals. Partners with management to develop tools and metrics to evaluate performance of target initiatives; assists with planning monitoring criteria and methodology.7.Ensures proper maintenance of documentation requirements. Ensures availability of performance measurement data for review and analysis. Evaluates and drives risk management process improvements, efficiencies and workflow enhancements.8.Partners with management, identifies end user education, training program and services; assists in the development and maintains department policies and procedures.9.Provides subject matter expert support in the development of clinical information systems and integration to improve the timely completion of projects.10.Operates under general guidance and work assignments are varied and require interpretation and independent decisions on course of action.11.Collaborates with internal stakeholders to maintain contact and cooperate with organizations for ongoing programs.12.Integrates the needs of outside groups with the goals, services and programs of the area and the organization.13.Provides logistical coordination for key visitors and guests.14.Performs related duties as required.
