Clinical Care ManagerLocation: Newburgh, NY, United StatesDate Posted: Aug 15, 2024Share: share to e-mail Apply NowSave JobSavedDescriptionLocation: Newburgh, NY Hours: M-F 830 am to 5 pm Salary: $63,000 (License required) OVERVIEW OF PRIMARY RESPONSIBILITES : Identify (eligible) high need, high risk individuals and provide a service approach that includes a focus on the clinical and medical coordination, understanding and addressing barriers to treatment, and an aim to improve member outcomes. This includes providing education to members (and their families) about their mental and physical health conditions, treatment options, and preventive care. Empower members to manage their health through self-care techniques and lifestyle changes. PRIMARY FUNCTIONS :
Provide HH+ care management services to eligible individuals (maximum caseload of 15).
Attend SPOA meetings, participate in discussions regarding HH+ referrals, and accept eligible cases.
Assist with HH+ intakes for new referrals.
Partner with other HH+ care managers on cases that involve a high level of medical and behavioral health needs.
Work closely with the HH+ Senior Clinical Coordinator to identify beneficial trainings for staff and assist in providing those trainings.
Identify and assess eligible individuals for a high level of care coordination services.
Ensure all HH+ members meet continued eligibility. Track eligibility for all HH+ members. Meet with the care manager 2 months in advance to assess for ongoing HH+ eligibility. Gather the necessary documentation of proof. Review with Senior Clinical Coordinator.
ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:
Assignments may be determined at a future date.
QUALIFICATIONS AND ATTRIBUTES :
Strong organization skills with accurate attention to detail.
Strong problem solving skills
The ability to clearly explain complex information, build rapport, and collaborate effectively.
Excellent communication skills and the ability to multi- task.
Clinical knowledge - Maintain an understanding of clinical issues and appropriate clinical interventions to address issues, including evidence-based interventions for the populations identified in the program.
EDUCATION AND EXPERIENCE :
LMSW
Working towards an LCSW licensure.
At least 3 years’ experience providing care management services for individuals with co-morbid disorders (mental health and substance abuse).
Basic computer knowledge.
Ability to effectively communicate (both verbal and in writing).
Excellent documentation skills.
Valid driver’s license required.
Access to personal car and able to independently travel in the community and between multiple agency locations.
PHYSICAL CHARACTERISTICS :
Must be capable to sit or stand in front of a computer for long-periods of time.
Able to work in open space floor plan.
Must be capable to move throughout work day and follow people served throughout community.
Work alongside co-workers within 3 feet.
