Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errandsCommunication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularlyDocumentation. Accurately and completely document client care on activity sheetCompliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirementsPHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.LICENSES/CERTIFICATIONS: HHA - Home Health Aide certification - New York State Education Department (NYSED)NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)PAY RANGE: $17.60 - $19.50The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future.
