Intake Worker Intake WorkerThe OpportunityFCA is excited to announce an opportunity for the position of Intake Worker. As we continue to grow and evolve, we are seeking a passionate behavioral health professional to join our dynamic team! The ideal Intake Worker will create a positive workplace culture, champion access to care, and play a pivotal role in our multidisciplinary team. This role requires an understanding of and commitment to quick rapport building with new clients.Specifically, the responsibilities and qualifications of the position include:Responsibilities
Conduct initial intake and comprehensive assessments, utilizing approved screening and assessment tools, including LOCADTR/ LOCUS, and collaborate with clinical team on treatment recommendations.
Responsible for the coordination of health assessments and psychiatric evaluations with appropriate medical staff.
Collaborate with external service providers and entities to ensure coordination of care and making referrals to outside providers, as necessary.
Responsible for explaining treatment process to participants, including the program consent for treatment, program rules and regulations, confidentiality, and other consent forms.
Maintains a listing of referral agencies with contact names and phone numbers and maintains contact with these agencies for ongoing recruitment and referral purposes.
Complete all agency, program and regulatory reports, correspondence and other required documentation and submit it by the required deadline
Function as an integral member of the inter-disciplinary treatment team and participate in case conferences, team meetings, and in-service workshops as scheduled
Assume other tasks, projects and responsibilities assigned to support the operations of the program
Qualifications Required:
Bachelor's degree in Human Service related major required or H.S Diploma/ GED and completion of appropriate CASAC coursework
QHP credential LCSW, LMSW, LMFT, LMHC, and/or CASAC; will consider candidates eligible for limited permit or CASAC-T/ CASAC-P
Minimum of 1 year experience providing screening and assessment to substance use or mental health population either working in an OASAS/OMH or similar program (may include internship).
Valid and clean New York State Driver's License
Preferred:
Bi-lingual plus
Compensation Information
The competitive compensation range is reflected with FCA's framework, culture, and values. Individual pay decisions will vary based on skills and experience' while still allowing for growth.
Additional Information
Salary: $45,000-$50,000
Schedule: (M-F), evening and weekend hours, as needed
This role is full-time (35 hours/week), Exempt
