Jersey City Medical Center is also ideally located in the vibrant Jersey City waterfront community overlooking the harbor, just 10 minutes from New York City, with a grand view of the Statue of Liberty.Job DescriptionProcesses all referrals into an identified program, includes: providing basic information to callers, recording the RIC, logging referrals and scheduling intakes. Maintains contact with the referal agent throughout the process to ensure continuity. Assists QA. Chairperson in preparing QA. Meetings.Additional RequirementsHigh School Diploma and one year of experience in behavioral health. Spanish speaking required. Responsible for responding to all telephone calls and walk-ins for referrals to the program. Completes the Record of Initial Contact on all referrals, collects all written materials. Advises the management of all problem/potential problem referrals immediately; takes action according to the manager's instructions. Assigns all intakes according to program policies and procedures; using the PHS, all intakes are scheduled for the next available appointment. Logs all RIC's electronically/manually for future reference, tracking, and TQM putposes. Manages the PHS using all module mctions to maintain the data base in an organized, continually updated fashion. Directs all visitors and clients to the assigned clinician. Places calls to applicant to remind them of their appointment. Prepares reports on all RIC's and intakes for the Program Director's analysis in an effort to monitor and demand and identi ' trends. Calls no shows to assess interest in services. Calls referral agents to advise on patient's status. Advises QA. Committee of all activities, including regular meetings, subcommittee meetings, report deadlines, and cancellations when necessary. Forwards to all interested parties, all minutes, reports, studies, and related materials in a timely fashion. Takes QA. minutes at all QA. meetings as requested. QA. documents are maintained in a file according to monthly meetings and are retrievable as needed. Monitors all QA. follow-up activity to ensure completion of all actions. Provides back-up assistance to the clerical staff as needed. Meets productivity expectations.
