The Home health assistant (CHHA) provides personal care and related services in the home. She/He functions under the direction, instruction, and supervision of the staff Registered Nurse, the Director of Nursing.
Responsibilities:
It is mandatory that the CHHA follow the client's Plan of Care, which is initiated and revised as needed by the RN. Daily documentation must be maintained with the CHHA’s signature, title, and date.
. To assist with ADL’s and IADL’s as stated on the Plan of Care without exception
.To follow the Plan of Care initiated by the RN
.Assist the client to maintain good personal hygiene
.Assist in maintaining a safe environment
.plans and prepare nutritious meals
.Taking and chatting vital signs as per the plan of Care
Responsibilities are not all inclusive. Will expand on duties upon interview and orien/training.
.Must have clear communication skills

Pay: $18.92 - $21.13 per hour
