You’ll be responsible for developing and overseeing personalised care plans that enhance our clients' independence and well-being.Key Responsibilities:Develop and Monitor Care Plans: Conduct initial and ongoing comprehensive assessments to create and update personalised care plans. Support customers by monitoring their health and clinical needs, and provide guidance while liaising with other agencies and healthcare professionals.Coordinate Allied Health & Wellness Services: Oversee wellness, reablement, and social support services based on assessed needs. This includes managing allied health services, organising multidisciplinary meetings, and handling related administrative tasksManage Service Delivery: Ensure that services are delivered according to each client’s individualised plan and budget, while adhering to program guidelines and standards.Support and Review: Collaborate with client’s and their support networks to regularly review and adjust care plans. Aim to maintain independence and ensure that care aligns with the client’s goals and program guidelines.Clinical Governance: Contribute to the clinical governance of the STRC program, ensuring that expenditures are managed effectively and align with client budgets.Collaborate with a Supportive Team: Work closely with a team of professionals to provide seamless, person-centered care, ensuring a cohesive approach to meeting each client’s needs.What We’re Looking For:Registered Nurse with relevant qualifications and experience.Passion for working with Older Australians and enhancing their quality of life.Strong organisational skills with the ability to work independently and as part of a team.Commitment to delivering tailored and compassionate care.Strong understanding of Aged Care Quality Standards, Open Disclosure, The Aged Care Code of Conduct, and SIRS.Proficiency in MS Office and Client Information Systems & Platforms (Carelink+ desirable).If you’re a Registered Nurse seeking a role that combines professional fulfillment with work-life balance, we’d love to hear from you.
