Seeking a caring, energetic, optimistic, and reliable Licensed Practical Nurse to join an organization that build's a world in which individuals with disabilities and other disadvantages realize acceptance. Job Description:Responsible for assessing and attending to member’s health concerns and issues, as well as developing and implementing therapeutic programs. The LPN ensures programmatic and contractual obligations are met. Duties include but are not limited to:

Prescribe medication dosages, routes, and frequencies based on patient characteristics such as age and gender at the order of Physician.

Develop treatment plans based on scientific rationale, standards of care, and professional practice guidelines at the order of Physician

Prescribe medications based on efficacy, safety, and cost as legally authorized by physician.

Counsel patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over-the-counter (OTC) medications, or herbal remedies as directed by Physician

Recommend interventions to modify behavior associated with health risks as directred by physician.

Educate patients about self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances.

Qualifications:

State of MA LPN License

Associate’s degree with LPN certification and 1-2 years related experience.

One year of supervisory or human service experience is desired.

Knowledge of and respect for diverse populations.

Ability to use computers to include Microsoft Word, data entry, email, and internet essential.

Valid driver’s license, with at least 3 years of driving experience, and a good driving record

Must be able to travel within the region to access residential settings, employers, and community organizations.
