OverviewWe at Fairview Health Services are looking for a dedicated Cultural Broker! This position will be full-time, mobile/remote! Pay beginning at $25.54, and is subject to increase, based upon experience and education level.Increase opportunities for community members to build stress-resilience and holistic well-being on St. Paul’s East Side as part of the East Side Mental Health & Stress Resilience Collaborative, a community-based collaborative sponsored by M Health Fairview. Work with the East Side Hispanic/Latino community, traditional and non-traditional providers, and community organizations to increase access to culturally based services and social supports; increase provider knowledge, cultural responsiveness, and collaboration; and decrease cultural stigmas associated with mental health. This position is hosted and located at CLUES ( Comunidades Latinas Unid as En Servicio) in South Minneapolis, with time spent in an elementary school setting.This is a 1.0 FTE (80 hours per two week pay period), benefits-eligible opening . Attend monthly partnership and team meetings.

Co-navigate school, health, and other mainstream systems with clients to foster self-sufficiency and reduce stress.

Develop and implement an outreach program to seek out and work with referrals and socially isolated individuals and families.

With identified partner organizations, shape this position to meet the unique needs of the focus Hispanic/Latino community.

Responsible for data collection, evaluation, and proper reporting.

Conduct visits for occasional resource support.

Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures, and standards.

Actively participates in creating and implementing improvements.

Patient Advocacy: Gathers information that reflects the patient's perspective; advocates and brings resolution to customer concerns; effective in mediating with third party payers on behalf of the patient.

Identifies problems; determines accuracy and relevance of information; utilizes appropriate tools and staff resources along with sound judgment to generate and evaluate alternatives, and to make recommendations.

Responsible to cover for other Cultural Brokers in the Hispanic/Latino Community, when necessary.

Responsible to perform work at multiple sites as needed.

Performs other responsibilities as needed/assigned.

Performs other duties as assigned.

QualificationsRequired Experience:

5 years of experience working in the Hispanic/Latino community, preferably in some aspect of social service and health.

Understands Hispanic/Latino tradition and cultural practices.

Experience working in a bicultural community.

Language proficiency and fluency in Spanish and English. Must be able to read and write in both.

Must have access to reliable transportation to perform work at multiple sites.

Knowledge and application of administrative and clerical procedures and systems such as word processing, managing files and records, and other office procedures and terminology.

Ability to think creatively.

Preferred Education:

Associate degree
