RN Case Manager, Fulltime, in a Home Health Care Agency. Job responsibilities include managing a case load of 30-36 patients, admit, discharge, transfer, resume and be knowledgeable of the patients healthcare status. Supervise LPN's and Home Health Aides, participate in care coordination with PT, OT, Speech and Medical Social Worker. On-call responsibility every 6 weeks. Function independently.
