Healthy Families Home Visitor

Healthy Families is a home visiting program that provides new, first time parents under 24 years old, with information on child development, support, and a link to community resources.Major Job Responsibilities include:Schedule, plan, and complete home visits with families on caseloadProvide parenting education to families using curriculum in the following areas: parent child interactions, parent and child safety, parent and child health, child development, anticipatory guidance and discipline, healthy relationships, healthy pregnancy, labor and deliverySupport participants in their roles as first time parentsComplete screening and assessments for the child and family, including developmental questionnaires and mental health screensDevelop a goal plan with each familyMaintain documentation related to work with families in electronic client recordComplete program required reports within established timelinesConnect families to community resources and provide appropriate referrals when neededOrganize, facilitate, and participate in group events for HF familiesParticipate in community outreach activities and maintain professional relationships with community partnersCollaborate with community agencies working with parents and children including the Department of Children and FamiliesPractice cultural humility when working with familiesAttend intensive home visiting and topic-based trainings provided by HFAAttend biweekly staff meetings, participate in case reviews and attend other team activities
