This role also conducts commitment and recommitment evaluations, as assigned.Key Responsibilities Provides individualized care and treatment, including psychotherapy, consistent with age and life span developmental needs of patients. Develops and updates bio-psychosocial assessments and treatment plans, and implements interventions to address barriers to discharge. Coordinates discharge planning meetings, develops release plans, and refers patients for services in the community. Conducts crisis intervention and assists families in understanding mental illness, including providing family support groups. Maintains documentation in compliance with standards, provides training and supervision, participates in quality improvement, and represents patient needs in meetings.Qualifications & RequirementsEducation
Master’s Degree and Licensure in Social Work or Mental Health Counseling is required.
Experience
(2) years of experience with the severe and persistent mentally ill is preferred.
Licenses/Certifications
Licensed Professional Social Worker; LCSW, LICSW or LMHC preferred. License eligible with combination of education and experience accepted.
