The registered Nurse coordinates all aspects of the client’s home care and interacts with other disciplines as needed. He/he provides professional care to the client, as indicated by the Agency policies, State and Federal laws/regulations. In home, admission visits and eligibility determinations are part of the RN’ duties, as is the development of the care plan. The RN also prepares paperwork necessary for Agency policies and federals/State laws and assists with the training and supervision of the Care Aides.

Qualifications
Is a graduate of accredited school of professional nursing and is currently licensed as RN through the MA Board of Nursing.
Has the minimum of one year’ nursing experience, experience, preferably with home care experience.
Possesses and maintain good physical stamina and mental health and can present a pre-employment health clearance.
Must be licensed driver with at least one-year clean driving record.
Has a vehicle that is insured in accordance with State/ or Agency requirements and in good working order.
Has excellent writing and charting skills.
Can pass a background check and drug screen.
Responsibilities/Activities
Assesses the home environment and the determine needs of clients
Records the health needs of the client.
Initiates Nursing or Care Aid’s Care Plan /or Homemaker/Companion service Pan and associated paperwork, in accordance with Agency policy.
Evaluate and revises the Care Plan, as needed.
Coordinate the Care of all assigned clients’
Prepares written instructions for the Care Aide.
Supervises the Care Aide in the home.
Instruct Care Aides in the proper use of practices, procedures and equipment.
Administration medications
Hang I.V s premixed with medications.
Hang hydrating fluid without medications.
Flush peripheral lines with saline or heparin solutions
Calculate and adjust flow rates.
Start peripheral I.V ‘s
Inspect insertion sites, change dressings, and remove peripheral I.V needles/catheter.
Respiratory care
Suction: Oral Nasal, Tracheal
Tracheostomy
Tracheostomy Care, Trach Change, HME, PM valve, Cap use
Ventilator/Ventilation:
BiPAP
Medication
Nebulizer use, Oxygen (type of delivery system),Pulse Oximeter Use.
Airway Clearance
Chest Physiotherapy (CPT),Cough Assist, Inexsufflator
GastroIntestinal (GI)
G-Tube, Feeding Pump, Managing Reflux, Syringe Bolas, Aspiration Precautions, and Venting G-Tube
Wound care
Wound Care, wound assessment.
Musculoskeletal
Use of Orthotics, Splints
Medication Administration
G-tube
Durable Medical Equipment

Job Conditions
The ability to communicate well, verbally and in writing, in English, is required.
The ability to access patients’ homes, which may not be routinely wheelchair accessible, is required.
Hearing eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and turning of the patients.
Equipment
Utilization of calculator, Ulti-line phone, copy machine, basic medical equipment.
Company information
Has access to all patient medical records, personnel records, and patient financial accounts, which may be discussed with Executive Director, Director of Reimbursement, Employee Relations, and Quality Assurance department staff.
Qualifications
Graduated from an accredited School of Practical Nursing.
Must be licensed in the State of Massachusetts as a License Practical nurse.
One, or more, years in Home Health Agency or in a hospital setting. Home health experience preferred.
Must have a current CPR certification. Online certification is not accepted.
