We are seeking an experienced LPN/Director of Nursing to oversee the medical and psychosocial needs of our rest home residents. We are a non-profit Corporation providing room/board and care to residents with psychiatric conditions. Qualifications require the candidate will have graduated from an accredited school of nursing, is currently licensed in Massachusetts in good standing with the board of nursing and have 5 years of clinical experience. Experience working in long term care and with mental health clients preferred. LPN license with no disciplinary action on license. The ideal candidate will possess strong leadership skills and have a comprehensive understanding of nursing administration, ensuring our nursing staff adhere to best practices in patient care, documentation and compliance. This role requires a strategic thinker who can manage nursing operations effectively while fostering a collaborative work environment.
Responsibilities include but are not limited to:
Oversee the medical and psychosocial needs of all rest home residents
Review records on a regular basis, update order/medication sheets monthly
Communicate with MD/NPs for order changes/changes in level of care, Psychiatrist, Therapists and social worker
Assess residents monthly for any new needs or issues
Provide assistance with transporting and escorting residents to appointments as needed
Staffing Responsibilities
Monitor and evaluate the CNA/RPs who care for our residents
Provide DPH required in-services and education to CNA/RPs
Observe staff on all shifts on a regular basis
Educate, monitor and discipline staff as needed and appropriate
Complete a 3 month and or annual review (merit based for annual pay raise if applicable on all CNA/RP staff)
Hire and onboard train new CNA/RPs including but not limited to medication administration, proper use of controlled substance
Log and Report book, personal care, laundry, appropriate and professional interaction with residents, de-escalation techniques,
what, when and how to report incidents, how to send a resident to the emergency dept, etc.
Ensure all hiring paperwork is filled out and in the employee file
Administrative duties
Maintain medical records up to date and charts debrided on a regular basis
Quarterly notes on all residents
Complete all Masshealth PT-1 forms for residents
Compliance/scheduling all vaccine clinics
Shared administrative coverage with the Executive Director
Screening and admitting of new residents
Discharging, re-leveling of current residents
Ensure compliance with all DPH guidelines
