Assist members with activities of daily living and personal care
Ensure that the environment remains clean, safe, and free of hazards for all clients and staff members
Actively monitor and ensure that clients follow all safety protocols, especially during physical activities or group programs
Communicate effectively with the healthcare team, coworkers, clients, and their families regarding any issues, progress, or concerns with participants in the day program
Serve meals and assist with feeding
File paperwork in client charts
Take monthly (or as needed) Vital signs and weights and effectively communicate any issues directly to the nurse manager
Administers prescribed medications or treatments only under the supervision of a licensed nurse or healthcare provider. All medication administration must follow proper protocols, including verifying prescriptions, documenting dosages, and monitoring clients for any adverse reactions
Restock all medical supplies including adult briefs, diapers, and sanitary pads
Assist the nurse manager with entering client information into patient portals
Transporting members to and from the day program
Transporting members to and from community outings
Assist with kitchen duties as needed
Other duties as assigned
