Clinical Supervisor, Hospice Home Care

OverviewOversees, supervises, coordinates, facilitates and supports Nurses and extended Care Team. Provides clinical and field supervision of assessments and care coordination interventions, including but not limited to, Telehealth, Health Risk Assessments and post-discharge assessments. Supervises the day-to-day clinical and operational health related technology (i.e. Telehealth and other web-based applications) as a standard of care, integrating virtual data in clinical decision-making and care delivery processes. Ensures assessments and care coordination is provided with the standards of nursing practice. Engages in the development, refinement, and didactic content necessary to support clinical decision making. Works under general supervision.
