Position Summary:

The Triage Associate will assess new patient referrals for appropriate level of mental health care. Performs phone screenings and in-person assessments and arranges/recommends follow-up care. Provides short-term, solution-focused counseling to a subset of patients

Qualifications:

EDUCATIONAL REQUIREMENTS Master's Degree in Social Work or related field CERTIFICATION & LICENSURE REQUIREMENTS Licensed Professional Counselor Illinois upon hire or Licensed Social Worker (LSW) Illinois upon hire. EXPERIENCE REQUIREMENTS One year (1) year experience providing mental health assessments, case management and/or crises intervention, and triage services. SKILLS AND KNOWLEDGE Must have strong communication and listening skills. Ability to work effectively with a variety of personalities at various organizational levels. Must have the ability to remain calm in crises situations and make clear decisions about patients that would best serve patients. Familiarity with working within an EMR. Strong documentation skills. Understanding of current DSM diagnostic criteria. Comfort working with patients from a variety of backgrounds and presenting problems. Ability to work both independently and collaboratively within a clinical setting.

Essential Functions:

Assesses new patient referrals. Determines the appropriate level of mental health care based upon phone screen (outpatient psychotherapy, medication evaluation/management) based on phone screening and/or in-person assessment.

Provides short-term, solution focused counseling to a subset of patients.

Provides education to patients, families and group

Documents accurately and completely.

Screens new patients referred to the Psychiatry/Psychology Department for treatment, and refers new patients for appropriate services inside and outside the Department.

Completes appropriate documentation in medical record within time frames established by departmental policy.

With permission of patient, provides communication back to referring providers about patient diagnosis, treatment plan, and progress

Maintains patient confidentiality and fulfills provider responsibilities as specified under HIPAA, the Illinois Mental Health and Developmental Disabilities Code, and the Illinois Mental Health and Developmental Disabilities Confidentiality Act.
