Position Summary:

The Home Care Scheduling Coordinator performs centralized scheduling for nursing, therapy, and home health aides. Schedules and assigns visits based on patient need, staff expertise and geographic location. Strives to maintain equitable work load and enable staff productivity goals. Works closely with referral intake to achieve maximum efficiency and timeliness in scheduling admissions. Hours may vary depending upon census and program need.

SKILLS AND KNOWLEDGE Strong computer skills. Interpersonal skills. Ability to successfully work with demanding personalities and meet internal/external customer needs without becoming involved emotionally. Able to work independently to prioritize visits: determine effective routing to optimize the time of professional staff while minimizing travel time and distance. Performs centralized scheduling for nursing, therapist and aides. Ensures continuity of communication, documentation, and required record keeping. Ensures work load and staff productivity are met. Open communication with direct 1-up, field staff, Home Care Manager, and home care supervisors.

Essential Functions:

Answers phones in accordance with customer service standards and directs calls to appropriate person.

Maintains centralized scheduling function for Nursing, Therapy and Home Health Aide visits.

Assigns admission visit, primary nurse and daily visits based on most effective and efficient use of staff resources; considers future need of patient, skill of nurse, geographic locations of staff and patient.

Monitors staff productivity and overtime hours; ensures staff is scheduled according to standard and overtime is kept to a minimum.

Maintains ongoing working (draft) schedule for current plus one week for use as worksheet to determine staff availability for assignments.

Keeps clinical manager and case managers advised of daily schedule needs. Notifies manager of clinical hours that can be reduced due to low scheduled patient visit for following day.

Coordinates closely with referral intake, advising of availability for new admissions for next 2-3 days to ensure smooth transition into Home Care.

Monitors all discharge and cancellation communications and removes unneeded visits from future schedules.

Provides administrative, secretarial, and clerical support for department including but not limited to: workque and work list tracking, scheduling, patient registration, intakes, supply room management, and other job functions as assigned by management.

Assist leadership team with staff schedule to facilitate productivity, mileage stewardship, and reduce in overtime.

Completes monthly staff schedule for nursing including on-call assignment. Enters on-call assignment into system for home care and Home Health Aides.

Answer telephone, take and deliver messages according to department communication standards.
