The Care Coordination Lead NWR reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics, the Corporate Compliance Program, policies & procedures, guidelines and all other regulatory and accreditation standards.Under the supervision and in collaboration with the Manager of Care Coordination, this role oversees general department operations, serves as a role model, builds trusting interdisciplinary relationships while engaging in conflict management, coaching, mentoring, and staff development.Responsibilities:50% time direct patient care coordination/caseload50% time project management/team oversightCoordinates daily assignments based on staffing, including re-direction of resources to critical areas dictated by circumstances and staffing patterns; flexing their own caseload to meet the needs of our patients and team.Provides support, guidance, and suggestions to team members by overseeing and problem solving for our Complex Case Management patient population. Steps in to assist as needed to ensure facilitation of timely discharge while meeting patient care requirements.Collaborates with the Manager of Care Coordination to provide the team with a supportive environment that fosters team-work, ongoing professional development, and personal health and well-being.Provides ongoing feedback related to team members performance to the Manager while coaching and mentoring staff.Provides input into Associate reviews and attends annual evaluations as indicated. Actively participates in the interview, orientation, competency assessments, and performance management processes for the team.Assists the Manager in overseeing and ensuring the quality and outcomes of work performed by the team, providing team support when indicated to achieve established goals. Ensures team members operate in accordance with departmental processes and current standards of the Joint Commission, as well as all federal, state, and regulatory governing bodies.Assists with team meetings - scheduling, agenda development, and facilitation.Organizes and facilitates in-service training for team members.Maintains open communication with the Manager of Care Coordination and keeps Manager apprised of important issues and concerns that arise on the team.Works in collaboration with Utilization Review to maintain efficient and seamless processes of the Care Coordination department as a whole.Project Management involvement including leading or participating in performance improvement initiatives as directed by following an established performance improvement model.Works collaboratively with Northwestern stakeholders to identify opportunities for improvement and implement best-practice solutions in areas such as patient, physician, and associate satisfaction, clinical outcomes, and financial performance. Facilitates gathering, analyzing, and presenting data in order to monitor outcomes.Performs other duties as assigned by the Manager of Case Management.
