HourlyPosition Purpose:
Responsiblefor delivering of a range of activities for individuals who are enrolledin the health plan for Medicaid or and Medicare in order to impactindividual health outcomes and provide assistance to the clinical team ofnurses and social workers
Activitiesinclude, but are not limited to outreach, community education, informalguidance and member support
Educate,coach and support members to understand disease prevention and achievegood health outcomes, including diabetes, high blood pressure, mentalhealth, substance use, etc.
Participatein coordination and resolution of medical and non-medical needs, includingappointment tracking, documentation of member information, referrals andfollow up, facilitating transportation to services, etc.
Participatein meetings with external providers and community organizations to buildpartnerships for our members to be able to leverage member care services
Providekey information to providers for improving members care based on member’shome environment and communities
Conductnon-clinical general health assessments in order to refer members toappropriate services, resolve concerns on member’s behalf, and gatherinformation for medical providers and staff working within theorganization
Conductnon-medical assessments such as home safety, assessment of thecommunity/environment resources, transportation, employment, and others tobe able to refer to appropriate services, resolve concerns on member’sbehalf, and gather information for medical providers in staff workingwithin our organization
Coordinateand implement community events such as baby showers, health fairs, andother health education events
Conducttelephonic and in-person outreach to locate individuals and families inthe community who are hard to reach
Workwith other community health workers internally and externally to sharebest practices, strengthen education and outreach
Participatein large scale community assessments including resource mapping, communitysurveys, and community meetings to discuss findings and resolutions to keymember concerns
Makefrequent visits to individual homes and community organizations
Education/Experience:
Highschool diploma or equivalent. 2+ years of community health, social work,social services, community advocacy, community outreach, member services,or education, experience
Understandingof the community in designated region through shared experiences or strongdesire to help people in vulnerable communities. Bilingual skillspreferred
Licenses/Certifications:
Validdriver’s license and proof of insurance
CommunityHealth Worker Training/Certification must be successfully completed within15 weeks of hire date
