Care Coordinator (Remote)
Schedule: Part-time; Monday/Wednesday/Friday (8am - 5pm)
Job type: Temp; Remote (Oahu residents only)
Pay Rate: $20/hour
About the Role:
The SUD CARES (Substance Abuse Disorders, Coordinated Access Resource Entry System) Care Coordinator will provide intake, phone, referral, and clerical services for the SUD CARES Referral and Treatment Call Center. The Care Coordinator will provide real-time, effective information and referral services and intake screening to anyone in Hawai’i in need of substance use treatment and mental health support/services.
Essential Duties and Responsibilities:
Answer incoming phone calls, text messages, email correspondence, and live-web chat requests to provide information and screening for individuals seeking substance use disorder treatment, recovery services, co-occurring disorders, and other behavioral health needs.
Complete a client profile, intake, and screening
Provide supportive counseling, crisis assessment, screening, and coordination of client services based on urgent mental health or substance use needs, including case management for clients in crisis, coordinating care with clients, case managers, and ADAD treatment facilities to create effective wrap-around treatment support for individuals in crisis.
Utilize motivational interviewing, trauma-informed principles of care, and other modalities to assist with de-escalation and warm-hand offs to appropriate treatment provider
Review and verify applicable agency and program requirements for referrals based on needs and available services; approve for entry into applicable programs in response to client needs
Assess and review pre-treatment services are completed for all client self-referred
Participate in quality assurance checks with CSAC Sr. Care Coordinator and/or Clinical Supervisor to perform client follow-up, with a sample of callers, to ensure clients receive appropriate services that are effective.
Conduct clinical review and assessments based on clients individualized behavioral health needs, including co occurring substance abuse and mental health disorders as necessary.
Sufficient knowledge on program procedures and requirements for providing accurate and quality service
Additional Functions:
Deliver high-quality, accurate and effective information and referrals for SUD and mental
health services.
Perform data entry on updates and new resources.
Informed on program safety and emergency response protocols
Performs other duties as assigned.
Education or Experience:
High school diploma or GED required, some college course work in social work, human services, public administration, or public health highly preferred
At least 1 year of direct service experience working in social/human services/non-profit organizations (not necessarily related to SUD/Mental Health Coordination)
Previous working experience in client services via nonprofit organizations or social service providers, preferred
Demonstrated ability with a variety of computer applications, including MS Office and database software, Voice over IP (VOIP) telephone, desirable.
Must have, or be able to get, CPR/First Aid training for certification.
Must provide and maintaining documentation of an initial and annual tuberculosis (TB) skin test or chest x-ray.
