Summary of Program and Position:
Family Center provides a wide array of services to individuals and Families living in Kalihi and surrounding areas. Community members are assisted with basic needs such as food, clothes, household goods, and school supplies. The Family Center also provides support by linking individuals to needed resources.
The Family Support Worker (FSW) works to engage clients participating in the Family Centers by providing service coordination and supports. The FSW will provide case and service coordination for the Family and Economic Opportunity Centers. This position is under the direct supervision of the Program Manager.
Essential Job Duties:
1. Behaves in a manner that supports the agency’s mission and follows personnel policies and procedures.
2. Completes intakes and assessments for clients and assists with accessing services through the Family Center and partner programs.
3. Facilitates and organizes groups, workshops and community events; collaborating with community partners and providers as needed to fulfill and support the goals of the programs.
4. Work with community partners, make appropriate referrals, coordinate service delivery, and maintain positive relationships.
5. Assesses each family’s strengths and challenges, identifies their needs and develops goals and objectives as part of the Family Success Plan; conducts on-going review of goal plan.
6. Assists in providing outreach services to identify, recruit and maintain positive working relationships with all parents and community members.
7. Provides case and service coordination, ongoing support for client plans, and conduct at least monthly visits at clients’ homes, the office, or in the community. The visit will be culturally sensitive, strength base approach and of the clients choosing.
8. Completes clear, concise, and non-judgmental written and electronic documentation, such as initial assessments, contact with clients or partners, and reports, and ensuring that all paperwork is completed and submitted in a timely manner.
Minimum Qualifications:
Education/Training/Experience:
-Bachelors’ Degree in counseling, education, social work, or related field, preferred.
-Six (6) months experience working with families in a social service or health care setting.
Other:
-Possesses and maintains a valid driver’s license.
-Has daily access to an automobile.
-Possesses and maintains proof of no-fault insurance.
-Can obtain a State Criminal History check upon initial hire, one year later, and every other year -thereafter or as requested.
-Can obtain an acceptable CPS child abuse registry check upon initial hire, one year later, and every other year thereafter or as requested.
-Can obtain an acceptable Sex Offender registry check upon initial hire, one year later, and every other year thereafter or as requested.
