Ensures appropriate coordination among all professional and paraprofessional services related to the care provided to the patient. Responsibilities:
Completes an initial assessment of the patient and family to determine home care needs.
Completes a comprehensive physical assessment of the patient.
Assess home safety and makes suggestions when hazards are noted.
Arranges for services/ supplies ordered and manages the coordination of other professional
Required license or certification:
NEW GRADUATES (ASN&BSN) WELCOME TO APPLY
Home care experience preferred
Excellent verbal and written communication skills
Proficient in the use of Microsoft office Suite tools
