INTERVIEWS are Tuesdays (1pm to 4pm) Fridays (12:30 to 3pm)
REQUIREMENT TO APPLY: MUST have completed LEVEL 2 BACKGROUND WITH AHCA

- DRIVER'S LICENSE OR FLORIDA ID
- SOCIAL SECURITY CARD
- If applicable,please bring with you:
- RESIDENT CARD
- CITIZENSHIP
TRAININGS
- CPR CARD
- HIV/BLOODBORNE PATHOGEN
- PROFESSIONAL LIABILITY INSURANCE
- All other certs that you have, please bring with you.
You will need at least 1 hour to complete your application. Please do not bring children.
If your certifications are expired, we will provide the training onsite for $10.00 per certificate training. CPR does not apply. Inquire about CPR training cost upon arrival.
